Consumer Health Data Notice

This notice describes how the Institute for Founder Dynamics ("IFD," "we") collects, uses, and shares consumer health data through the Cofounder Diagnostic (the "Diagnostic").
1. Categories of health data we collect and why
Categories.
- Answers about your physical and mental health and wellbeing as they affect your work, for example stress, emotional challenges, health conditions, sleep, eating habits, access to medical care, and use of professional support such as a coach or therapist.
- Answers about how your cofounder handles stress or is at risk of burnout.
- Scores and report content derived from these answers.
Purposes and how we use it.
- To produce your joint report. These answers feed the scores for areas of your partnership such as resilience, stress, and conflict.
- To check that reports are accurate and to improve the Diagnostic.
- For research using de-identified data.
2. Sources
We collect this data directly from you, through your answers to the Diagnostic. Some data about you also comes from your cofounder's answers about you. Derived scores and report content are created by us from those answers.
3. Categories of health data we share
We share the categories listed in section 1, only with the recipients in section 4 and only for the purposes in section 1.
4. Who we share it with
- IFD staff and contractors, including coaches who work with teams for IFD. Access is limited to those who need it for the purposes above, and all are bound by confidentiality.
- A coach who invited you to the Diagnostic, who can see your individual answers, scores, and report, only if you agreed at signup.
- Service providers that host and process data for us: hosting, database, sign-in, and email providers.
- Research collaborators at universities and research institutions: de-identified data only.
- Government authorities or others, where the law requires it.
We do not sell consumer health data.
5. Your rights and how to use them
You have the right to:
- confirm whether we collect, share, or sell your consumer health data, and access it, including a list of the third parties we have shared it with;
- withdraw your consent to our future collection and sharing of it;
- have it deleted. We delete what identifies you, and may keep de-identified data that cannot reasonably be linked to you.
To make a request, email info@founderdynamics.org from the email address on your account. We may ask for information needed to confirm your identity. We respond within the time the law requires.
Appeals. If we decline your request, you can appeal by replying to our decision and asking us to reconsider. We will respond to your appeal within a reasonable time. If we deny your appeal, we will tell you how to submit a complaint to the Washington State Attorney General or your state attorney general.
